Hammersmith & Fulham We are dedicated to providing a wide range of services in Hammersmith & Fulham to empower our service users with safety, hope, and purpose. Our support offer includes the following services. Mental health We offer a wide variety of mental health support services in Hammersmith & Fulham: The Mental Health Housing and Support (North Cluster) and Mental Health Housing and Support (South Cluster) services offer accommodation-based support for adults with enduring mental health needs. This includes low-medium and medium-high needs. The Harwood Road service is a registered care homes for adults with mental health needs who have been referred by Social Services. The Hammersmith and Fulham Mental Health Community Support service provides support for anyone ages 18+ with a mental health need, who lives in or is registered with a GP in the borough. Hammersmith and Fulham Safe Space is an inviting, and inclusive space for individuals who are struggling to cope with their mental health and daily life. Housing Our Housing Support & Inclusion Floating Support service offers short-term housing support for people aged 16+ across the borough. Employment support Hammersmith & Fulham residents over the age of 18 have access to a variety of employment support through Twining-Hestia: Our West London Works programme supports those who have mental or physical health challenges. Our Developing Futures programme supports young adults (aged 18-24) from ethnically diverse communities looking for personalised and realistic advice on how to achieve their life goals. Modern slavery response Our modern slavery service started in 2011 and we have supported more than 7,000 survivors since then. We provide seven safe houses in London and Kent, as well as a pan-London outreach service working in every London borough. Need help in an emergency?
